Comprehensive Medical Rehabilitation Program

Our Comprehensive Medical Rehabilitation program provides daily medical management and treatment for patients who need rehabilitation services following a surgical procedure or for a condition that affects normal functions, such as:

  • Amputation
  • Congestive heart failure
  • Post-operative cardiothoracic surgery
  • Pulmonary conditions including:
    • COPD
    • Respiratory failure
    • Restrictive lung disease
    • Post-operative lung reduction
  • Oncology (medical and surgical)
  • Total joint replacement and spinal decompression

Our team of physicians, nurses, and physical and occupational therapists will:

  • Coordinate your care
  • Develop a personalized treatment plan
  • Help establish your rehabilitation goals
  • Determine an appropriate completion date for your rehabilitation

Depending on your needs, speech therapists, recreational therapists, respiratory therapists and clinical dietitians may also participate in your care.

While no two patient treatment plans are exactly alike, most plans feature a combination of therapies and activities designed to improve your physical mobility and other abilities needed for daily living. If needed, we can arrange additional consultations for conditions related to care specialties such as pain management, podiatry, wound care and ENT.

Rehabilitation Case Management

An important aspect of our rehabilitative care is case management. Our case managers help keep your family informed about their progress as well as help you prepare for discharge from our program. Case managers also serve as the point of contact for insurance issues, working closely with both you and the insurer to find and address any potential issues or concerns.
